We need someone that has the same vision as us! Someone that always keeps the patient’s goals and needs in the back of their heads at all times! Using a client centered approach; you will plan, develop and coordinate services for NDIS customers by managing staff to:

• Liaising with internal and external services to maximse participant outcomes
• Plan, develop and build capacity in participants to manage their own supports
• Listen, understand and provide mentorship to participants of NDIS
• Monitor and review service plans and goals in consultation with participants on an ongoing basis
• Provide regular feedback and reporting to NDIA and actively participate inthe review of the package as required
• Establish and develop relationships with key NDIS and health networks,community agencies and services.
• To be successful in this position, you will require outstanding language,communication and interpersonal skills, the ability to prioritise your workload and possess advanced knowledge of the Microsoft suite of products and web-based applications.

Previous experience as a Plan Manager coupled with a qualification at Degree level in a relevant health/social science discipline is required for the position. Please note if you do not possess a Bachelor Degree, we cannot consider you for these positions.
